In the chronic care management program, you will have 24/7 access to providers to address urgent needs along with community and social services.
How do I qualify for CCM?
To qualify for CCM, you must have two or more chronic conditions. For example, high blood pressure, high cholesterol, diabetes, heart disease.
How do I sign up for CCM?
To be enrolled in a CCM program, all you will need to do is call your providers' office and they will gladly schedule you an appointment to get started.
