Chronic Care Management includes any care provided by medical professionals to patients who have chronic diseases and conditions.  CCM places a focus on a continuous relationship with a designated member of the patient's care team.

Support to achieve your health goals.  Chronic Care Management can lower the risk of hospital stays, emergency room visits, urgent care visits, and nursing home admissions.  You will receive a faster response time to questions and concerns you may have with your healthcare needs.  Through CCM, you will also have one-on-one access through your care coordinator, for your specialty provider, assistance with monitoring your medications, and pharmacy needs. In the chronic care management program, you will have 24/7 access to providers to address urgent needs along with community and social services.

How do I qualify for CCM? 

To qualify for CCM, you must have two or more chronic conditions. For example, high blood pressure, high cholesterol, diabetes, heart disease.

How do I sign up for CCM?

To be enrolled in a CCM program, all you will need to do is call your providers' office and they will gladly schedule you an appointment to get started.
